If you run >50 concurrent agents 24/7, Twilio/Vonage math hurts. On-prem Asterisk pays back in months, not years.
2. When you need absolute dialplan control
Business-specific IVRs (digit routing, custom AGIs, legacy DB integration) are trivial in Asterisk and painful in the cloud.
3. When your client demands data sovereignty
Hospitals, government and banking. Period.

If your volume is low and variable, if you don't have SRE in-house, or if you just want a simple PBX for 5 people. There, Twilio Flex or RingCentral are the right choice.
